How to start in fullscreen and keep dvbdream focused

I'm sorry if this is a stu*** question, or been asked a million times, but I can't find the answer anywhere. I can't seem to find the option to start dvbdream in fullscreen mode, and I can't find the option so it always stays on top. I am using the newest version, in a media center computer.

DD Help > Others > Command line switches: dreamstart.exe /fullscreen

Right click on TV window > Stay on top.
