H.264 Jitter probelm

coreavc for h264 and nvidia for video and audio

The kitter is very high at 230ms the 180ms and down to 30ms and the hd h264 pic is not smooth. What is it causing this?
4400 X2 Athlon 2gig ddr2 ram, 8800 gt 512pci-e
Vista 32bit w/Genpix.Always trouble with h264.
