two questions:
1) what's up with all these supersized bold fonts? can't you use a normal font as everybody else?
2) why don't you mention important stuff, your system configuration, for example?
3) log file shows that custom (non-BDA) tevi diseqc commands are working fine: "OEM diseqc method: SUCCEEDED". Why do you think they are not working?

for instance, you should run Windows 7 to be able to use IBDA_DiseqCommand (Microsoft does not support such interface on any older OS).
there are two possible reasons of such error in your log:
1) you are using Windows Vista, or even older version of Windows
2) the driver for your tevi 660 is very old, download the latest driver from your manufacturer

on a side note: if tevi sends diseqc commands but your swicth is not responding to them, check you dish/coax-cable grounding.
in 95% cases, intermittent diseqc switching is due to incorrect antenna grounding.

clearly, there is a difference between win xp and win 7 (if you read the logs).
But this is important for cards which don't have custom *.ext extensions.
Since DVB Dream has tevi.ext, it should work on both windows versions.

You still did not answer: what makes you think that DVB Dream does not control your switch?
There are no errors in the log file about diseqc commands, but you are claiming that it does not work.
If switch does not work as you expected (and assuming that your dishes aligned properly and there is a signal on those transponders which you try to catch), did the thought of "bad switch" ever come to your mind?
have you tried another switch?

I assume that you already proven that there is a signal on those TPs (you connected those LNBs to your card directly, bypassing the switch), right?

I assume that "Only 2 port is identified" means that you can get the signal only on LNB2 input.
but when you switch to another inputs, you get no signal.
And you tried that with two different switch models!
Do I understand you correctly?

the fact that you have different results when you switch from one port to another does prove that switch is working.

There is one obvious reason for that:
there is no signal on other inputs.
Did you ever think about that? what did you do to verify that signal is there?
Did you connect those LNBs directly to your card without a switch?

There is another obvious reason:
looking at your log files I see that it takes 3 seconds for your tevi card to get a lock on those transponders (which have "Locked" status in the log). And this is at SR = 30Msps!
Just for comparison: most DVB-S/S2 cards can tune such signal within 200-400ms (at least, always below half-second).
May be something's wrong with your tevi card?
Or may be you are overloading your card (some heavy load on coax cable does not allow your tevi card to provide enough LNB voltage)?

PS: I can not find the specs for your diseqc switch anywhere. Do you have a link?
if this is a type of switch which passes DC voltage to all connected inputs, it may explain why your tevi card is overloaded.

if you can not answer a simple question (can you get a signal or not, using your tevi card bypassing the switch), hardly anybody can help you, sorry.
